LUMBAR CANAL STENOSIS L2-3, L3-4 WITH EXTRUDED DISC L3-4 WITH LEFT DOWNWARD
MIGRATION.
Anamnesis
The patient came to the clinic 26-July-2014
complaining of left sciatica for 6 months with LBP.
The condition continued to deteriorate and the
last 2 weeks got bilateral sciatica. MRI lumbar
spine done 26-July-2014 showing old extruded
disc L3-4 left side with severe stenosis of L2-3
and L3-4 with old spondylolisthesis L4-5 and
very shallow disc space of this level. The
patient is a known diabetic for 5 years with
arterial hypertension for 14 years with CVA one
year ago.
On examination; the patient is limping with exaggerated scoliotic stance
walking bended anterior.
SLRS was 40 degrees with pain in the left
shooting to the right leg. There
is weak dorsiflexion both feet -3/5.
The patient was sent for investigations: MRI
lumbar spine done 27-July-2014 showing the same
data as before. Dynamic studies were normal. ESR
was 38 mm/h.
Laminectomy of L3, lower 3/4th of L2 and upper
half of L4 was done. Foraminotomy left L4 root
was achieved. Routine closure of the wound.
The patient showed good improvement of the power
of both feet.
Comments
The patient has lumbar canal stenosis
with progressive course. Surgical decompression is the only
solution.
