01-JUNE-2003 NASER HAMDAN ABU-SAMRAH 17 YEARS
CUT LEFT SCIATIC NERVE MED THIRD AFTER GUN SHOT.
Anamnesis
The patient came to the clinic 25-May-2003
complaining of complete no function of the left
sciatic nerve after gun shot 02-May-2003 to the
left thigh with fracture bone, for what plate
and screws were inserted at that time. ECS and
EMG done 25-May-2003 reporting complete
non-function f the left sciatic nerve. The exit
region of the bullet still oozing, for what
antibiotics were started.
The patient was operated at
Istiklal hospital. Projectional incision over
the left sciatic nerve. The nerve exposed above
the biceps muscle and below it. The maximum scar
was found under the biceps muscle with neuroma.
The neuroma was cut from both sides until the
fascicles of the nerves were seen. Direct
anastamoses was achieved after mobilization of
the nerve above and below. The anastamoses was
lax with the knee in flexion position. Routine closure of the
wound. A cast for 8 weeks was applied to keep
the knee in flexion position.
Smooth postoperative recovery.
He was sent to the ward.
FOLLOW UP
He came 09-March 2004
with recovery of the left soleus and gastrocnemius
muscles and shrinkage of the hypalgesia. there is no
painful neuroma. The patient was advised to undergo
physiotherapy and medical treatment but he escaped.
Comments
The patient has complete cut of the sciatic
nerve and recovery will need long time.
