08-MAY-2007 NASER MUHAMED ZREQAT 44 YEARS HUGE EXTRUDED DISC L5-S1 WITH
RIGHT DWONWARD MIGRATION AND RIGHT COMPRESSION.
Anamnesis
The patient came to the clinic 23-April-2007
complaining of right sciatica for 7 years,
progressing in frequency.
On examination: the patient had scoliotic stance
with painful SLRS 80 degrees in the right .
There is weak dorsiflexion right foot.
MRI lumbar spine performed 25-April-2007 showing
extruded disc L5-S1 with wide base and right
downward migration.
Bilateral foraminotomy of S1 roots was performed
and the right root was adherent and woody in
consistency. The extruded downward migrating
piece was removed from under the right axilla.
Bilateral cleaning of the disc space, which was
narrow and degenerative. The right S1 root
became lax and the woody consistency was due to
the woody extruded disc, which was removed in
three pieces. Routine closure.
Smooth postoperative recovery.
Comments
Recurrence in this case must be less than
usual, because the disc space was narrow.
Meticulous bilateral cleaning decrease the rated of recurrence.
