The patient  was complaining of severe LBP for more than 7 months  with left sided sciatica . He came with almost drop foot and scoliotic stance and hypalgesia of the left L5 root territory. The patient was operated and left L4-5 hemiflavotomy with left L5 foraminotomy was done. The extruded disc was removed in one piece and minimal cleaning of the disc space was performed from the defect in the annulus fibrosis, from where the extrusion came out.
