DISC C5-6 AND BULGE C6-7.
Anamnesis
The patient from Dubai came to the clinic 25-October-2003
telling that he was exposed to RTA 1991 with
fracture hip and was operated for the fracture
hip. The last year start to complain of LBP with
right sciatica. There discrepancy of the length
of the lower limbs. The left lower limb is
longer than 15 mm. The patient start to complain
of neck pain with exacerbation last 2 weeks with
right upper limb pain.
On examination: the patient has pain when
looking to all directions with weak grip
and extension right hand 3/5 and right triceps
3/5. Hoffman positive both sides. All the deep
reflexes lower limb exaggerated. There is weak
dorsiflexion left foot 4/5.
The patient was sent for investigations and MRI
cervical spine showing huge extruded disc C5-6
with bulge C6-7 with malacia of the spinal with
compression at C5-6.
Through anterior approach, discectomy of C5-6
was done until the dura was seen along all the
posterior aspects. The C6-7 was only
decompressed. All stages of surgery were
done under C-arm control.
Routine closure of the wound.
Smooth postoperative recovery. The power of the
right upper limb became normal and the radicular
pain dramatically
decreased.
FOLLOW UP
The patient
came to the clinic 04-December-2003 with clean wound
and the power of the right upper limb improved, but
still having slight weak extension right hand.
Comments
The recurrence rate in
cervical disc is zero.
Decompression of the C5-6 was performed to lower
the incidence of recurrence or escalation.
