40 YEARS RESIDUAL AFTER SEVERAL FAILED ATTEMPTS OF TRANS-SPHENOIDAL
REMOVAL OF PITUITARY ADENOMA WITH SLIPPED LPS.
Anamnesis
The patient was operated by me for pituitary
adenoma
18-August-2002 after several failed attempts
for trans-sphenoidal removal complicated by CSF
leak and insertion of LPS to stop CSF leak,
elsewhere. The patient started to complain of
abdominal and right flank discomfort, for what
investigation were done.
On examination, the patient is neurologically
free but complaining of abdominal discomfort.
The patient was sent for investigation and MRI
sella showed complete radical resection of the
pituitary adenoma. The lumbo-peritoneal shunt
slipped completely to the peritoneal cavity.
Through the same abdominal
wound, the peritoneal cavity was reached and
using finger, the shunt was found and removed.
Routine closure of the wound.
Smooth postoperative recovery.
She was sent to the ward.
FOLLOW UP
The patient
came 04-July- 2003
with clean wound and no complains.
Comments
The lumbo-peritoneal shunt is not needed and it
slipped completely to the peritoneal cavity.
Removal is better to avoid future possible
complications.
