50 YEARS EXTRUDED DISC L5-S1 LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic
13-August-2002
complaining of LBP for 2 weeks with left
sciatica down with numbness left L5 territory
with positive cough sign.
She has epi activity under treatment for 2
years and right sided migraine for what MRI
brain done 20-January-2002 was normal. MRI done 02-August-2002 showing
extruded disc L5-S1 with left foraminal
occlusion.
On examination: The patient in agonizing pain,
limping with exaggerated scoliotic stance.
SLRS
70 degrees left side with pain. There is weak
dorsi and planterflexion left foot 3/5 with
hypalgesia left L5 and S1 territories.
Left S1 foraminotomy with removal of the
extrusion lateral to the axilla. Left sided
intradiscal cleaning L5-S1. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak.
Routine closure of the wound.
Smooth postoperative recovery.
The power of the left foot improved.
She was sent to the ward.
Follow Up
The patient came to the clinic 25-August-2002
with clean wound and full power of the left foot
with no hypalgesia.
Comments
There is still an estimated postoperative
recurrence around 7% because the disc space is still not
shallow.
