30-JULY-2003 NIDAL YOUSEF SAEED 32 YEARS
EXTRUDED DISC L2-3 WITH SEGMENTAL STENOSIS.
Anamnesis
The patient came to the clinic
09-July-2003 complaining of LBP for 1 month with
bilateral sciatica, down to the knees with
numbness of the left foot. Numbness of the right
upper limb with neck pain for 1 year and
diagnosed with bulge C4-5. MRI lumbar spine
performed 02-July-2003 showing wide-based
extrusion L2-3.
On examination, the patient is limping with
exaggerated scoliotic stance. SLRS was 70
degrees both sides with pain. There
is weak dorsi and planterflexion both feet 4/5
more the right.
Decompressive laminectomy
L2-3 after leveling the site with C-arm with
foraminotomy both L3 root. The extruded disc was
removed lateral to the right axilla and
right sided intradiscal cleaning L2-3 was achieved.
Inspection the left side was negative for
extrusion. The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. Routine closure of the wound.
Smooth postoperative recovery.
The patient showed improvement of the power of
his feet.
He was sent to the ward.
FOLLOW UP
The patient
came to the clinic 10-August-2003 with clean wound
and SLRS 80 degrees without pain and normal motor
and sensory functions.
The patient
then came 11-October-2003 with mild pain right hip.
He was treated conservatively.
Comments
There is still an estimated postoperative
recurrence less than 7%, because the disc space
is shallow, but not completely collapsed.
