EXTRUDED DISC L4-5 RIGHT SIDE.
Anamnesis
The patient came to the clinic
03-May-2004 complaining of LBP with right
sciatica for 5 months increasing in intensity. MRI
lumbar spine done 29-April-2004 showing huge
extruded disc L4-5 with right foraminal
occlusion.
On examination: The patient is limping with
exaggerated scoliotic stance. SLRS was 75 degrees
in the right with pain. There is almost drop
right foot with weak planterflexion same foot
3/5.
Foraminotomy
both L5 roots with
removal of the extrusion lateral to the axilla
from both sides. After
that, the roots became lax and bilateral
intradiscal cleaning L4-5 disc space was done.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. A free fat grafts
harvested from the subcutaneous layer
transferred to the roots area to
minimize postoperative scarring. Routine closure
of the wound. Smooth postoperative recovery. The
power of the right foot became normal. She was
sent to the ward.
FOLLOW UP
The patient came to the clinic 18-May-2004
with clean wound and SLRS 90 degrees both sides
without pain and no motor, nor sensory deficit.
Comments
The estimated postoperative recurrence is still
ranking around 7%, because the disc space is
still relatively not shallow.
