AL-SAYYED 50 YEARS WIDE BASED HUGE EXTRUDED DISC L2-3.
Anamnesis
The patient was operated by me for PCD C3-4, 4-4
and C5-6 with fusion with Scientx plate for
C3-4-5-6 09-March-2002, then came to the clinic 04-October-2003 complaining of LBP for
1 month with bilateral sciatica.
On examination: the patient is in agonizing
pain, limping with exaggerated scoliotic stance.
SLRS was 60 degrees both sides with pain. There
is weak dorsi and planterflexion both feet with weak both quadriceps muscles.
The patient was sent for investigation and MRI
lumbar spine performed 05-October-2003 showing
huge extruded disc L2-3 wide based.
Decompressive laminectomy
L2-3 with foraminotomy both L3 roots .The very huge piece was removed from
under the right L2 axilla. Right sided L2-3
discectomy lateral to the
axilla and right sided intradiscal cleaning of
L2-3 space was achieved. Bilateral cleaning of
L2-3 was achieved. The epidural fat was
transferred around the L2 root to minimize
postoperative adhesions. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak.
Routine closure of the wound. Smooth
postoperative recovery. The power of both feet
dramatically improved.
FOLLOW UP
The patient
came to the clinic 24-October-2003 with clean wound
and the power of both lower limbs normalized. SLRS was
80 degrees.
The patient
then came 17-April-2016 telling that the last year
having upper limbs pareasthesia. CT-scan cervical
done 13-April-2016 showing calcified OPLL. The
patient was sent for investigations and MR cervical
spine done 02-May-2016 showing severe cervical canal
stenosis due to ossified OPLL. She was advised for
posterior decompressive laminectomy. but she
disappeared.
Comments
The patient with
age progressed OPLL causing cervical canal
stenosis, which needs posterior decompression.
The estimated postoperative recurrence of lumbar
disc surgery is around 7%, even with bilateral
cleaning.
