EXTRUDED DISC L4-5 RIGHT AND L5-S1 LEFT SIDE.
Anamnesis
The patient came to the clinic
26-August-2002
complaining of LBP with left sciatica since
11-June-2002. CT-scan done 20-August-2002
showing PLD L4-5 right side and L5-S1 left side
with calcification.
On examination: The patient is limping, has LBP with
exaggerated scoliotic stance. There is weak dorsiflexion
right foot
3/5 and planterflexion both feet 4/5 and SLRS was
90
degrees with pain both sides more the right.
The patient was sent for investigation and MRI
lumbar spine showed extruded disc L4-5 right
side and L5-S1 left side.
Laminectomy L5, upper edge of the
sacrum, lower third of L4 with right L5 foraminotomy
with removal of the extrusion lateral to the axilla
and drilling of the osteophytes with right sided
intradiscal cleaning of L4-5 disc space. Left S1
foraminotomy with left sided cleaning of L5-S1 with
osteophytectomy lateral to the axilla. The patient
was put in Reverse Trendelenburg position with
Valsalva maneuver and hyperventilation. No CSF leak.
Routine closure of the wound.
Smooth postoperative recovery.
The power of the left foot normalized. She was sent to the ward.
Follow Up
The patient came to the clinic 17-October-2002
with clean wound and hypalgesia left S1 root
territory and weak dorsiflexion right foot 4/5.
Comments
The estimated postoperative recurrence is
around 7% to each disc, because the disci spaces are still not shallow.
