EXTRUDED DISC C5-6 MORE TO THE RIGHT.
Anamnesis
The patient a Saudi citizen came to the
clinic 17-August-2004 complaining of neck pain
with cephalgia and both upper limbs pain with
numbness both hands.
On examination, the patient
in agonizing neck pain, having neck pain when turning
the head to the left. There is severe weak
grip and extension right hand 3/5 and right
triceps muscle 3/5.
The patient was sent for
re-investigation and MRI of the brain
done19-August-2004 showed small scattered
lacunar infarcts and huge PCD C5-6 more to the
right.
Using the C-arm, the level of
C5-6 was identified and discectomy of C4-5 was
performed and the extruded pieces compressing
the spinal cord were removed until the dura was seen at the
bottom of the removed disc cavity. Routine closure
of the wound.
Smooth postoperative recovery. The power of the
right upper limb dramatically improved.
FOLLOW UP
The patient
came to the clinic 28-October-2009 with clean wound
and neurologically free. MRI cervical spine done
26-August-2009 showing fusion of C5-6. She was
complaining of signs of CTS.
The patient
then came 16-August-2014 complaining of LBP with
bilateral sciatica, more the left for 5 months with
weak dorsiflexion both feet. MRI lumbar spine was
done 03-May-2014 showing bulge L4-5. The patient was
given medications and sent for investigations, but
she did not return.
Comments
The recurrence rate
in cervical disc surgery is zero. Another disc
can cause de novo extrusion.
Skyra MRI with all clinical applications in the run since 28-Novemeber-2013.
Inomed Riechert-Mundinger System, with three point
fixation is the most accurate system in the market. The microdrive and
its sensor gives feed back about the localization.
