The above mentioned patient was complaining of agonizing pain shooting to the left lower limb for several weeks and severe weakness of both lower limbs . The patient operated 26-March-2005 with complete laminectomy and bilateral foraminotomy of both S1 roots, after what the extruded disc was cleaned from both sides. When the extrusion is wide-base and in old patients, it is recommended to attack the extrusion from both sides to assure proper cleaning of the extrusion. The cause of the left sciatica was explained by more elevation of the extrusion and rough surface of the disc material compression the left S1 root.
