24-JULY-2007 OEDEH RAMADAN SALEM OEDEH 36 YEARS MEDIUM SIZED EXTRUSION L5-S1
RIGHT SIDE.
Anamnesis
The patient came to the clinic 13-December-2006
complaining of LBP for 2 months with right
sciatica.
MRI performed 18-October-2006 showed a medium
extruded disc L5-S1 with downward migration left
side.
On examination: the patient is limping and has
scoliotic stance with SLRS right side 65 degrees
with pain and absent AJ right side. There is
weak dorsi 3/5 and planterflexion 4/5 right
foot.
The patient came another time 05-February-2007
with new MRI showing a small extrusion L5-S1
touching the right S1 root. Conservative
treatment was advised.
The patient then came 01-May-2007 with
deterioration of his condition and was sent for
another MRI.
The patient came 11-July-2007 with new MRI
urging for surgery.
Hemiflavotomy L5-S1 with right S1 root
foraminotomy was done. Most of the extrusion was
removed lateral to the axilla. Meticulous
cleaning of the disc space was performed from
the right side. Routine closure of the wound.
Smooth postoperative recovery. The power of the
right foot improved, but the patient still
complaining of sciatica, but with lesser degree.
Comments
The disc space was medium in height, which
predict a medium rate of recurrence.
