19-OCTOBER-2015 QASEM ABBAS NAJEM 40 YEARS
HUGE EXTRUDED DISC L4-5 MORE TO THE LEFT WITH SEVERE SEGMENTAL STENOSIS.
Anamnesis
The patient was operated by me
23-February-2012 for extruded disc L3-4. The
patient then came 09-December-2013 complaining
of LBP without sciatica and check MRI ruled out
recurrence of the extrusion and he was treated
conservatively. The patient then came
28-March-2015 complaining of left hip pain with
SLRS was 40 degrees with pain in the left. MRI
performed 28-March-2015 showed small extruded
disc L4-5 and conservative treatment was
advised. The patient then came to the clinic 18-October-2015
complaining of left
sciatica for 1 month with inability to walk the
last 12 days with numbness big toe left foot. He
is using 2 crutches for walking the last week.
On examination: the patient was not limping at
the moment of examination. SLRS was 10 degrees
with pain left side. Weak dorsiflexion left foot
2/5 with hypalgesia below the left knee.
Decompressive laminectomy
L4-5. Foraminotomy left L5 root. The huge
central extruded disc was removed from the left
side with left sided cleaning L4-5 disc space.
Routine closure of the
wound.
Smooth postoperative recovery. The power of
the left foot showed slight improvement. The patient was
sent to the ward.
Comments
The patient still has an estimated
postoperative recurrence around 7%.
