BURST FRACTURE OF L1 WITH BONY COMPRESSION.
Anamnesis
The patient from another hospital after
suffering RTA 22-June-2015 without loss of
consciousness with mid back pain. MRI lumbar
spine done 23-June-2015 showing burst fracture
of L1 with bony fragment inside the spinal
canal.
On examination; the patient is in bed with pain
in the mid back. Neurologically he was free.
There is pain when palpating the injured part of
the spine with localized edema of the area.
Skeletonization D12 down to L3 with preservation
of the intraspinous ligaments. 4 polyaxial
6.5x40 legacy screws inserted to D12 and L2.
2 straight rods 10 cm inserted and by trying to
distract the left side, the lower screw shifted
down, The rods removed and another 6.5x50 mm
monoaxial screws inserted to L3 and to the left
pedicle of L1 5.5x50 mm. 2 rods 15 cm length
bended inferior to conform the anatomical
alignment were inserted. Step-wise reduction
with distraction was applied in several points
to achieve acceptable reduction of the slipped
bone fragment. Cross connector was applied at
the center of the rods. All stages of surgery were guided
using the C-arm. Routine closure of the wound.
Smooth postoperative recovery.
Comments
The patient has unstable fracture with
bony compression which could harm the spinal cord after
ambulation. Surgical fixation and reduction is the only way
to avoid such possible complication.
Polyaxial screws cannot stand stress
distraction and in this case it is recommended to use the
rigid monoaxial brand.
