The patient came to the clinic 02-July-2003 complaining of LBP for 8 years with exacerbation of LBP  the last 2 months with left sciatica the last 5 days. Thyroidectomy done 1990 in no treatment.

On examination, the patient is limping with exaggerated scoliotic stance. SLRS was 30 degrees right side and 25 degrees left side with more pain. There is almost drop left foot with weak planterflexion 3/5 with hypalgesia left L5 and S1 territories.

Left L4-5 hemiflavotomy with foraminotomy left L5 root. The extruded disc was removed lateral to the left axilla and left sided intradiscal cleaning L4-5 was achieved. The patient was put in Reverse Trendelenburg position with Valsalva maneuver and hyperventilation. No CSF leak. Routine closure of the wound.

Smooth postoperative recovery. The patient showed improvement of the power of her left foot. She was sent to the ward.

The patient came to the clinic 12-July-2003 with clean wound with SLRS right 80 degrees without pain and 45 degrees left side without pain. There is o motor or sensory deficit.

The patient came several times and MRI lumbar spine performed 26-June-2005 showing no recurrence, but she is complaining of swollen right knee.

The patient the last time came 13-November-2014 with many complains and MRI cervical done 12-November-2014 showing bulges all disci and MRI lumbar spine showing spondylolisthesis L3-4 with segmental stenosis. Dynamic studies showed II degree spondylolisthesis L3-4. She was advised for fixation and decompression, but she disappeared.

There is still an estimated postoperative recurrence around 7%, because the disc space is not shallow.

Over the long follow up the patient can suffer from different problems not related to his surgery, but deep in his brain he think there is connection.
