35 YEARS EXTRUDED DISC L4-5 CENTRAL WITH RIGHT FORAMINAL
OCCLUSION.
Anamnesis
The patient came to the clinic 20-April-2014
complaining of difficult walking for three
years, more during winter time. The last 6
months got LBP and right sciatica. MRI lumbar
spine done 07-January-2014, bad quality showing
central bulge disc L4-5. MRI lumbar spine
repeated 08-April-2014 better quality, but not
complete study, showing extruded disc L4-5
ventral with right foraminal occlusion.
On examination; the patient is limping with
exaggerated scoliotic stance. SLRS was 40
degrees with pain in the right. There is weak
dorsiflexion both feet -4/5. MRI of the lumbar
spine repeated the same day confirming the
previous data.
Using the C-arm, the L4-5 level identified.
Using MICRODISCXS system, foraminotomy right L5
root was performed and the huge extruded disc
was removed lateral to the axilla. Right sided
cleaning L4-5 disc space. The root hanging free.
Routine closure of the wound.
Smooth postoperative recovery. The power of the
both feet became normal.

Comments
The patient has still have an estimated
postoperative recurrence around 7%, because the
disc space is still not shallow.
The MICRODISCXS set helps as good retractor
system, but lacking illumination and as seen in
the video, nothing is seen inside the wound.
