04-JUNE-2014 RAED ABEL-SALAM AL-JAABARI 36 YEARS
HUGE EXTRUDED DISC L5-S1 WITH LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 01-June-2014
complaining of LBP with left sciatica for 20
days. MRI lumbar spine done 27-May-2014 showing
huge extruded disc L5-S1 with left far downward
migration.
On examination; The patient is not limping
during the examination, with mild scoliotic stance. SLRS was
80
degrees in the left without pain. There is weak
dorsi and planterflexion left foot 4/5.
The left AJ was absent.
Using image-intensifier, the L5-S1 level was
identified. Bilateral flavotomy of L5-S1 and
foraminotomy left S1 root was achieved. The
extruded disc was removed subaxillary and left
side cleaning of L5-S1 disc space was done.
Routine closure of the wound.
Smooth postoperative recovery. The power of
left foot became normal.
Comments
The estimated postoperative recurrence is
still around 7%, because the disc space is still not
shallow.
