EXTRUDED DISC L5-S1 WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 27-November-2014
complaining of LBP with left sciatica the last 2
months down to the III-IV toes left foot with
numbness. MRI lumbar spine bad quality done
11-October-2014 showing extruded disc L5-S1 with
left foraminal occlusion. The patient had trauma
4 years ago with fracture of the pelvis with
fracture both bones left leg and left foot. He
was treated conservatively at that time.
On examination, the patient is using crutches,
limping and with exaggerated scoliotic stance.
SLRS was 10 degrees with pain in the left. There
is weak dorsiflexion left foot -3/5 and
planterflexion 3/5. There is hypalgesia below
the left knee.
Left S1 foraminotomy with partial flavotomy. The
extruded disc was removed in one piece lateral
to the axilla. Left sided intradiscal cleaning
L5-S1 space. Routine closure of the wound.
Smooth postoperative recovery. The
power of the left foot became normal.
Comments
The patient still has an estimated
postoperative recurrence around 7%.
