16-OCTOBER-2007 RAED MURSHED MASWADEH 39 YEARS 39 YEARS PCD C5-6 WITH
RADICULO-MYELOPATHIC SYNDROME.
Anamnesis
The patient came to the emergency of
Al-Shmaisani hospital 15-October-2007
complaining of neck and both shoulders pain more
in the left with radiculo-myelopathic syndrome.
MRI performed 22-July-2007 showing PCD C4-5 and
C5-6 and C6-7 with prominent extrusion of C5-6
and malacia of the spinal cord at this level.
On examination: the patient had quadriparesis
with weak both upper limbs more the right and
lower limbs more the left.
Using Inomed Highline ISIS, the surgery was
performed under TES-MEP control. Discectomy of
C5-6 was performed and removal of the posterior
longitudinal ligament was performed, after what
further extrusion was removed.
Smooth postoperative recovery. The power of four
limbs normalized after recovery.

Comments
The patient has severe compression at
C5-6 and mild stenosis above and below. It was decided to
decompress the spinal cord at C5-6, leaving the other
levels. If, in the remote future these findings became
evident, then this will be corrected accordingly.
The disc extrusion had a protracted
course, which predict relatively persistent pain in the long
postoperative course.
Inomed Highline IOM TES-MEP mode was used
in this case.

fixation is the most accurate system in the market. The microdrive and
its sensor gives feed back about the localization.
