EXTRUDED DISC L5-S1 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic
17-July-2012
complaining of LBP with right sciatica for
1 year with intermittent course. Exacerbation
the last 2 weeks with occasional numbness of the
right foot.
MRI lumbar
spine done 14-July-2012 showing extruded disc
L5-S1 with right downward migration.
On
examination, the patient is not limping with
exaggerated scoliotic stance. SLRS was 90
degrees in both sides. There is weak dorsi and
planterflexion right foot 4/5. The AJ is
diminished in the right side.
Right S1 foraminotomy. The
extruded disc was not contained and it was
extruding under the axilla. It was removed in
several pieces from under the axilla, after what
the root became lax. The disc space of L5-S1 was
cleaned from the right side.
Routine closure of the wound. Smooth
postoperative recovery. The power of the right
foot became normal.

Comments
The patient still have
postoperative estimated recurrence around 7%,
because the disc space height still not shallow.
