28-DECEMBER-2003 RAHMAH MAHMOUD SMOOR 53 YEARS SEVERE CERVICAL CANAL STENOSIS C3-4, 4-5, 5-6
WITH OPLL.
Anamnesis
The patient came to the clinic 01-December-2003
complaining of neck pain for 2 years with both
shoulders pain and left radicular pain with numbness both hands with difficult walking.
The condition is progressing. The patient is a
known diabetic for 6 years.
On examination: the patient has pain in the neck
when looking to all directions. There is
hypalgesia right upper limb. There is weak grip and extension both
hands and both triceps muscles. Weak dorsi and
planterflexion both feet more weak left.
The patient was sent for investigations and MRI
cervical spine done 22-December-2003 showing PCD
C3-4, 4-5 and C5-6 with OPLL.
Using C-arm, discectomy of
C3-4, C4-5 and C5-6 was done until the dura was
seen at the entire posterior border of the
bodies. Using 3 level Scientx cervical plate fusion of
C3-4-5-6 was done.
Routine closure of the wound.
Smooth postoperative recovery. The power of four
limbs became better.
FOLLOW UP
The patient came to the clinic 08-January-2004
with clean wound and no sensory, nor motor
deficit.
The patient then came twice for neck pain and
check X-rays were acceptable and neurologically
free.
The patient then came 20-March-2006 complaining of
LBP with left sciatica with weak dorsi and planterflexion both
feet more the left. She was sent for cardio consultation and
thorough investigations.
Comments
The patient has
severe stenosis due to extruded disc C3-4, 4-5
and C5-6. Decompression and fixation is the best
solution for this situation.

fixation is the most accurate system in the market. The microdrive and
its sensor gives feed back about the localization.
