HUGE EXTRUDED DISC L4-5 MORE TO THE LEFT.
Anamnesis
The patient came to the clinic
19-August-2002
complaining of LBP with left sciatica for 20 months after
jumping with exacerbation last 8 months. MRI done 27-August-2002 showing
huge extruded disc L4-5 more to left.
On examination: The patient in agonizing pain,
limping with exaggerated scoliotic stance.
SLRS
60 degrees right side with pain and 10 degrees
with more pain in the left. There is weak
dorsiflexion left foot 3/5 with hypalgesia left
L5 territory.
Left sided L5 foraminotomy with removal of the
extrusion lateral to the axilla. Left sided
intradiscal cleaning L4-5. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak.
Routine closure of the wound.
Smooth postoperative recovery.
The power of the left foot improved.
He was sent to the ward.
Follow Up
The patient came to the clinic 09-September-2002
with clean wound and full power of the left foot
with normal sensation.
Comments
There is still an estimated postoperative
recurrence around 7%.
Skyra MRI with all clinical applications in the run since 28-Novemeber-2013.
Inomed Riechert-Mundinger System, with three point

its sensor gives feed back about the localization.
