YEARS EXTRUDED DISC L5-S1 LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic
27-October-2002
complaining of LBP with left sciatica for 5
months with exacerbation last month and numbness
left S1 territory.
On examination: The patient is limping, has LBP with
exaggerated scoliotic stance. There is weak dorsiflexion
left foot
3/5 and planterflexion left foot 4/5,
hypalgesia left S1 territory and SLRS was 45
degrees with pain left side.
Left S1 foraminotomy with
removal of the extrusion lateral to the axilla and
drilling of the osteophytes with left sided
intradiscal cleaning of L5-S1 disc space. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak.
Routine closure of the wound.

Smooth postoperative recovery.
The power of the left foot normalized. She was sent to the ward.
Follow Up
The patient came to the clinic 09-November-2002
with clean wound and normal power and sensation
left foot. SLRS became 80 degrees without pain
in the right.
Comments
The estimated postoperative recurrence is
around 7% because the disc space is still not shallow.
The video recording is very bad quality,
mostly the camera-man was sleeping.
