The patient came to the clinic 19-October-2003
with parents telling that he was born with
hydrocephalus and VPS inserted to him at 2 weeks
age in 13-June-2003. During labor with forceps
with IVH. Cytomegalovirus positive. The parents
noted enlargement of the head last 2 weeks.
On examination: the patient is sleepy, with soft
lax abdomen and normal peristaltic sounds. The
fontanel is tense. H/C 45 cm in comparison to
41.5 cm 1 week ago. Palpating the shunt was
indicating that the ventricular end is occluded.
The patient sent for investigations and the
ventricular system is large with the shunt not
broken.
The shunt reservoir was
exposed and the ventricular part, which was
yellow (brand from another company?) was removed
gently so as to avoid escalation of IVH.
Inspection of this part showed a clot occluding
the ventricular part. New ventricular part from
PS Medical was replaced and inserted through the
same tunnel. Routine closure of the wound.
FOLLOW UP
The patient
came to the clinic 01-November-2003 with clean wound
and lax fontanel with improvement of his neurologic
status.
Comments
The VPS has several
problems and one of them using different parts
from different companies. Even when using the
complete set from one company, different
problems could arise among them exposure of the
patient to infection with subsequent
ventriculitis.
