SEVERE DORSAL CANAL STENOSIS D10-11 AND D11-12.
Anamnesis
The patient is my grandmother in law was
operated by me 13 years ago for extruded disc
L5-S1 for right sciatica and recovered. She
underwent CABAG operation 2 years ago. Then she came 07-July-2004 complaining of
back pain with inability to walk for 3 months
with progressing course.
On examination: The patient is limping
with exaggerated scoliotic stance. SLRS was 80
degrees both sides without pain. There weak all
muscles both lower limbs.
The patient was sent for investigations and MRI
lumbar and dorsal spine done 07-July-2004 showing severe
stenosis D10-11 and D11-12.
Decompressive laminectomy D11
and upper half of D12 and lower half of D10 with foraminotomy both
D11 and both
D12
roots was done. The disci of D10-11 and D11-12 were
inspected and right side removal of the
extrusion of D11-12 was achieved. Check for
instability was negative. The subcutaneous fat
was harvested with pedicle and transferred to
the dura to decrease the postoperative scar
formation.
Routine closure of the wound.
Smooth postoperative recovery. The power of
both legs became normal.
FOLLOW UP
The patient
came to the clinic 27-July-2004 with clean wound and
no motor nor sensory deficit. SLRS was
90 degrees both sides
without pain.
Comments
The lumbar canal stenosis when not progressing
and the patient in good shape, we put him under
observation, sine progress and deterioration
happens, surgery is mandatory.
With age different pathologies arise and must be
dealt accordingly.
