IBRAHEEM ABU-SHHADEH 41 YEARS EXTRUDED DISC L4-5 WITH LEFT FORAMINAL
OCCLUSION.
Anamnesis
The
patient came to the clinic 07-May-2012
complaining of LBP for 7 years with
left sciatica intermittent course. Exacerbation
last week
with bilateral sciatica more the left. MRI
lumbar spine performed 17-April-2012 showing
extruded disc L4-5 with left foraminal
occlusion.
On
examination, the patient is limping with
exaggerated scoliotic
stance. SLRS was 45 degrees in the right and 30
degrees in the left, Dorsiflexion
left foot was -3/5 with hypalgesia left L5 and
S1 roots.
Left foraminotomy L5
root with preservation and reflection of the
ligamentum flavum to the right. The extruded
disc removed subaxillary and cleaning of the
L4-5 disc space was achieved also subaxillary.
The root was severely damaged by the
compression. Guardix-sol 1.5 was applied to
decrease the postoperative fibrosis.
Routine
closure of the wound. Smooth postoperative
recovery. Normalization of the power of left
foot.

Comments
The patient still having
estimated postoperative recurrence around 7%,
because the disc space is still not shallow.
