EXTRUDED DISC C5-6 RIGHT MIDIO-LATERAL.
Anamnesis
The patient came to the emergency of Shmaisani
hospital 4 days ago with agonizing neck pain and
radiating to the right upper limb.
MRI of the cervical spine performed 1 hour ago
showing a huge extruded disc C5-6 with right
midio-lateral localization.
On examination; the patient had severe
limitation of neck movement with severe weak
flexion and extension of the right hand and the
right triceps muscle. The patient was advised to
undergo surgery.
The patient came today, urging for surgery and
she was operated urgently.
Discectomy of C5-6 was performed through
anterior approach and after cleaning of the disc
space at C5-6, the defect of the posterior
longitudinal ligament was widened to make the
extrusion coming out and removed. Several
fragments were removed from the right upper and
lower corners. The dura was glistening relaxed
and check of the foramen was negative for
remnants.
Routine closure of the wound and smooth
postoperative recovery with disappearance of the
radicular syndrome.
Comments
The bony elements practically were
untouched and the single level all make unnecessary to put
artificial devices, such as artificial disc or fusion
devices of several types. Here the presence of Leuscko
joints in the lateral aspect of the disc space, provide the
stability, taken together with other supporting elements.
The good news in cervical disc surgery, that,
there is no recurrence in comparison to the lumbar disc which
still having recurrence rate around 7-10%.
