HUGE EXTRUDED DISC L5-S1 WITH RIGHT POSTERIOR MIGRATION.
Anamnesis
The patient
came to the emergency of Shmaisani hospital with
agonizing right sciatica for 2 weeks with
MRI lumbar spine done 06-February-2011 showing
huge extruded disc L5-S1 with right posterior
migration.
On
examination: The patient cannot find
proper position to relief his pain with SLRS 5
degrees in the right and drop right foot and
severe weak planterflexion right foot and
hypalgesia right S1 root territory.
The patient was taken immediately to the
operating room. Right partial flavotomy L5-S1.
The extruded subaxillary huge fragment was seen
pushing the right S1 root lateral. All the
extrusion was removed in several pieces to avoid
injury to the neural structures. The root
regained normal relaxed position and meticulous
cleaning of the L5-S1 disc space was was
achieved from the right.
Routine
closure of the wound. Smooth postoperative
recovery with normalization of the power of the
right foot.

Comments
The estimated postoperative
recurrence is still ranking around 7%, because
the disc space still not shallow.
