26-JANUARY-2012 RIYAD MAL-ALLA DAWOUD 45 YEARS
EXTRUDED DISC L4-5 WITH LEFT FORAMINAL OCCLUSION AND SECONDARY SEGMENTAL
STENOSIS.
Anamnesis
The
patient came to the clinic 11-January-2012 complaining of LBP for
27 years with left sciatica. The condition
deteriorated the last 2 months.
On
examination, the patient walking normally. SLRS
was 30 degrees with pain the left side. There is
hypalgesia below the left knee with weak dorsi
-4/5 and planterflexion 4/5 of the left foot.
MRI of the
lumbar spine was done 12-January-2012 showing
extruded disc L4-5 with left foraminal occlusion
with left downward migration.
Left L5 foraminotomy. The
left L5 root is severely compressed. The
extruded disc was hard in consistency and it was
removed partially lateral to the axilla and
partially subaxillary. Left sided cleaning of
L4-5 disc space. The root is hanging free.
Routine
closure of the wounds. Smooth postoperative
recovery with improvement of the power of the
left foot.

Comments
The patient is having estimated postoperative
recurrence around 7%, because the disc space
height still not narrow.
