SEVERE CERVICAL CANAL STENOSIS C3-4, 4-5 WITH TETRAPARESIS.
Anamnesis
The patient came to the clinic 19-October-2022
complaining of inability to walk for 40 days
with weak both upper limbs. The condition
started 1 year ago with slowly progressive
course. MRI of the brain done 17-October-2022
showing scattered lacunar infarction with
periventricular changes. MRI of the cervical
spine showing severe stenosis C3-4, 4-5 with
malacia of the spinal cord with compression more
from behind. The patient is a known hypertensive
for 4 years and diabetic for 4 years
On examination, the patient is in wheelchair
with tetraparesis both deltoids -4/5, both
biceps and triceps 3/4, flexion both hands 3/5
and extension 2/5. Both iliopsoas and quadriceps
femoris 3/5. Dorsi and planterflexion right foot
3/5 and 4/5 left foot. Right hemihypalgesia
below the face starting from right C3 dermatome.
Hoffmann positive left side. so Babinski left
foot. There is no neck pain.
The patient was sent for investigations and
dynamic studies ruled out overmobility of the
cervical spine. Seen by cardiologist and gave
permission for G.A.
Decompressive laminectomy
C3-4 and upper edge of C5. The dura was thin and
transparent. No CSF leak. The power of the
four limbs slightly improved. She was sent to the
ward.
FOLLOW UP
Too early now.
Comments
The sooner the decompression, the better
the outcome.
