The patient came to clinic 27-August-2014 complaining of LBP with bilateral sciatica for 2 months, more the left side with neck pain and numbness four limbs. The patient is a known diabetic for 12 years, underwent laminectomy for PLD L4-5 2006 cervical fixation C5-6 elsewhere. MRI lumbar done 15-June-2014, showing bulge L4-5. Cervical X-ray showing the cage between C5-6. EMG done 04-March-2014 reporting as having bilateral CTS, more the right.

On examination; the patient has shuffling gait with mild scoliotic stance. There is pain when turning the head to the right and down with limitation. There is weak grip both hands -4/5 more the right. Extension right hand 3/5, left hand 4/5. Triceps right arm 2/5,  and 3/5 of the left. There is hypalgesia of both hands with ulnar side of the arm and forearm. SLRS was 85 degrees with pain both sides. There is weak all muscles both lower limbs -4/5, more the left leg.

The patient was sent for thorough investigations and MRI cervical spine showed very huge disc C6-7 with spinal cord compression.

Discectomy C6-7 with removal of the all extrusion until the dura was seen behind. Samarys cervical cage 5x15x12 mm inserted. Using Trestle cervical plate one level 16 mm with four fixed 4x16 mm screws, fusion of C6-7 achieved. Routine closure of the wound.

Smooth postoperative recovery. The power of all four limbs became normal.

The patient has radiculo-myelopathic syndrome, which functionally resolved after decompression.
