SPONDYLOLISTHESIS L4-5 WITH SEVERE STENOSIS.
Anamnesis
The patient came to the clinic 30-October-2013
complaining of LBP for 15 years with bilateral
sciatica more the right with exacerbation last 8
months with inability to walk more than 100
meters.
On examination: the patient is limping with
exaggerated scoliotic
stance. SLRS was 75 degrees with tightness
both sides. There is weak dorsiflexion both feet
4/5 with hypalgesia both L5 territories.
MRI lumbar spine done 31-October-2013 showing
spondylolisthesis L4-5 with severe stenosis at
this level.
Laminectomy L3, L4 and upper
third of L5. Foraminotomy both L5
roots. The L5 lamina was completely sacralized
and the L4-5 segment was completely fused.
Inspection of the disc of L4-5 was done from the
right. There is a slight motion at L3-4 facet
due to severe degeneration. Bone
graft was used lateral to the lateral masses. Routine
closure of the wound.
Smooth postoperative
recovery.
Comments
The patient has stable spondylolisthesis L4-5.
Fusion was not needed in this case.
