PAIN AT THE OLD KYPHOTIC DEFORMITY AT TH8-10 WITH RIGHT SCIATICA.
Anamnesis
The patient came to the clinic 09-May-2015
complaining of mid back pain for long years
after spinal kyphotic deformity of the Th 8-10
since early childhood and right sciatica for
several years. MRI dorsal spine done
28-February-2015 showing the segmental kyphotic
deformity at Th 8-9 without any compression.
On examination; the patient is not limping. Neck and upper
limbs were neurologically free. SLRS was 90 degrees without pain. There is
hypalgesia right L5 root territory with weak dorsiflexion
right foot 4/5.
The patient was sent for investigations and MRI
lumbar spine performed 10-May-2015 showing bulge
L5-S1. Dynamic studies ruled out instability.
Using the C-arm, Inomed Neuro N50 with tcu 215
15 mm electrodes, radiofrequency stimulation and
ablation of Th8-11 and S1 facet rootlets was
achieved. The patient noticed immediate
disappearance of pain all over.
The patient discharged after 1 hour pain free.
Comments
RF ablation of the involved facets
generating the pain is gratifying procedure. It is done with
accuracy by testing with stimulation and simple. The
ablation time for each was 120 sec with temperature not
reaching above 90 degrees.
