29 YEARS VERMIAN MEDULLOBLASTOMA WITH MILD INVASION OF THE FLOOR OF THE VI
VENTRICLE LEFT SIDE.
Anamnesis
The patient came to the clinic 10-February-2003
complaining of headache, which increased the
last 5 months. CT-scan done 03-February-2003
showing posterior fossa mass.
On examination: the patient swaying to all
directions with double vision both sides. There
is hypalgesia right V1-2-3 divisions, right UL
and right S1 territory. There is left
hemiparesis. Babinski positive right side.
The patient was sent for investigations and MRI
of the brain showed vermian tumor fulfilling the
4th ventricle.
Osteoplastic craniotomy of the posterior fossa with
reflection of the bone flap to the bottom of the
field.
Approach to the vermis in the
midline and piece-meal resection was
performed. Fresh frozen sections gave the result
of medulloblastoma. Total resection of the tumor was achieved and
there was a dirt of the tumor at the left side
of the floor of the fourth ventricle.
Water-tight closure of the dura and the flap
retuned and sutured. Routine closure of the wound.
Smooth postoperative recovery. The patient was
kept for one day in the ICU.
Follow Up
The patient then came to the clinic
03-March-2003 with clean wound with same
neurologic signs before the surgery.
The patient performed MRI of the brain
20-March-2003 confirming radical resection of
the tumor, but he was advised to undergo
radiotherapy.
The patient then came 16-March-2004 telling that
he underwent radiotherapy with new MRI
performed 16-March-2004 confirming no recurrence
of the tumor.
The last time he came 26-November-2009 with new
MRI performed 26-October-2006 showing no
recurrence and improvement of his neurologic
status.
Comments
The medulloblastoma has good cleavage and can be
removed totally.
Skyra MRI with all clinical applications in the run since 28-Novemeber-2013.
Inomed Riechert-Mundinger System, with three point
fixation is the most accurate system in the market. The microdrive and
its sensor gives feed back about the localization.
