28-JANUARY-2007 SAEED UTHMAN AL-MASHARQAH 33 YEARS HUGE EXTRUDED DISC L5-S1
WITH LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 13-January-2007
complaining of LBP with left sciatica for 1
month with numbness of the IV and V toes left
foot.
On examination: the patient had scoliotic
stance, limping during walking with shooting
left sciatica. SLRS was 80 degrees in both
sides. He had weak dorsi and planterflexion left
foot 4/5 with hypalgesia left S1 territory.
MRI performed 15-January-2007 showed a huge
extrusion of L5-S1 disc with left downward
migration with reaction of the lower endplate.
Foraminotomy of the left S1 root was performed
and partial flavotomy of L5-S1 was achieved.
The extrusion was so big, part of it was removed
from under the axilla and part was removed
lateral to the axilla to avoid injury to the
running root. Meticulous cleaning of the disc
space was performed.
Smooth postoperative recovery and normalization
of the power.
Comments
The extrusion was massive and the disc
space was narrow and meticulous cleaning of the disc space
was achieved. The expected recurrence in this case is lower
than 7%.
The reactionary changes of the endplate could hide some sort
of inflammatory process. Time will show the importance of
this finding.
