BILATERAL EXTRUDED DISC L4-5.
Anamnesis
The patient came to the clinic 05-August-2014
complaining of LBP with right sciatica for 5 weeks and LBP for
3 months. MRI lumbar spine bad quality done
08-June-2014 showing extruded disc L4-5.
On examination; the patient is limping with exaggerated scoliotic stance.
Walking with help. SLRS was 50 degrees with pain in the
right. There is old drop feet with weak
planterflexion both feet 4/5.
The patient was sent for investigations: MRI
lumbar spine done 05-August-2014 showing
bilateral extruded disc L4-5 more to the left
causing severe segmental stenosis.
Using C-arm, the L4-5 level was identified and
foraminotomy both L5 roots was achieved.
Bilateral cleaning of the extrusion and
bilateral intradiscal cleaning of L4-5 disc
space. Routine closure of the wound.
Smooth postoperative recovery with dramatic
improvement of the power of both feet.
Comments
The patient still has an estimated
postoperative recurrence rate around 7%, even with bilateral
cleaning, because the disc space is still not completely
shallow.
