YEARS EXTRUDED DISC L4-5 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The
patient came to the clinic 01-October-2011
complaining of LBP with right sciatica for 1
week with numbness of the right foot. The
patient came several times 12-May-2009,
21-July-2011 and 23-July-2011 and he was treated
conservatively for bulge disc L4-5 and right
sciatica. The patient is a known diabetic with
cardiac problems, for what 2 stents were
applied.
On
examination: the patient is limping, has
exaggerated scoliotic stance. There is weak dorsiflexion
right foot
-4/5. SLRS was
35 degrees in the right side
with pain.
MRI lumbar
spine repeated 03-October-2011 showing extruded
disc L4-5 with right downward migration.
Right extended L5
foraminotomy was done. The far down migrating
disc was removed subaxillary in 2 big pieces.
Right sided cleaning of L4-5 disc space.
Routine
closure of the wound. Smooth postoperative
recovery with improvement of the power of
right foot and disappearance of numbness of the
right foot..

Comments
The estimated postoperative recurrence in this
case is above 7%, because the disc space is
still high.
