09-AUGUST-2015 SALAH ABDO AL-KHATEEB 68 YEARS
EXTRUDED DISC L3-4 WITH BILATERAL DOWNWARD MIGRATION WITH SEVERE SEGMENTAL
STENOSIS.
Anamnesis
The patient came to the clinic 01-August-2015
complaining of LBP with right sciatica for 3
months not reaching below the right knee. The
patient is a known hypertensive for 20 years in
medication.
On examination: He was limping with exaggerated scoliotic stance.
SLRS was 10 degrees with pain right
side. There was weak dorsi and planterflexion right foot
-4/5.
The patient was sent for investigations and MRI lumbar spine performed

showed extruded disc L3-4 with bilateral
downward migration and severe segmental stenosis. Dynamic studies
were negative for over mobility. He had gout for
what pain killers and Zyloric were prescribed.
The patient was sent for cardio and urologic
consultation.
Using the C-arm the L3-4 level was identified.
Decompressive laminectomy L3-4 with foraminotomy
right L4 root. Due to severe compression. there
was a dural defect at the axilla of the right L4
root lateral aspect about 1.5 mm. It was
repaired by nylon 4 zero. Discectomy L3-4 from
the right. The root became lax and free. A piece
of muscle was placed of the stitched dural
defect after performing the Valsalva procedure,
which was negative. Routine closure of the
wound.
Smooth postoperative recovery. The power of the
right foot became normal.
Comments
The patient still has an estimated
postoperative recurrence around 7% because the disc space
still not completely shallow.
