YEARS RSIDUAL AFTER HEAD INJURY WITH BONE DEFECT MID PARIETO-FRONTAL
REGION.
Anamnesis
The patient came to the clinic
08-August-2002 with bone defect after
decompressive craniotomy performed after severe
head injury with massive brain edema 2 years
ago.
On examination: The wide bone defect is in the
mid parieto-frontal region, disfiguring his
appearance and causing headache in
acceleration-deceleration situations. He is
afraid of suffering direct injury to his brain.
Skin incision over the bone defect area and the
flap reflected to the side. The edges of the
bony defect were refreshed so as accept the bone
cement. The bone cement was remolded to keep in
normal shape of the skull and to hinge with the
edges of the bone defect. The edges of the bone
cement then were drilled to make the parts
smooth and unnoticeable by feeling and the
unwanted surfaces were drilled of to have the
proper configuration. Routine closure of the wound.
Smooth postoperative recovery.
He was sent to the ward.
Follow Up
The patient came to the clinic 21-August-2002
with clean wound.
Comments
Decompressive wide craniectomy some tomes
help the patient survival after severe head injury with
massive edema and pending conning.
After several months, when the brain
swelling subsided, it is preferable to close the defect to
avoid further direct injury to the brain.
Bone cement application is an easy
procedure and reshaping is easy.
I used this method many times and never
met rejection of the material.
In my own cases I usually keep the
patient bone flap to be returned later in place.
