12-AUGUST-2003 SALI ORGANIDEEN AL-ALI 23 YEARS
HUGE CALCIFIED EXTRUDED DISC L4-5 WITH BILATERAL EXTRUSION.
Anamnesis
The patient came to the clinic
10-August-2003 complaining of LBP for 1 year with
bilateral sciatica. MRI
lumbar spine done today showing huge extruded
disc L4-5 with wide-based compressing both L5
roots.
On examination, the patient is limping with
exaggerated scoliotic stance. SLRS was 20
degrees both sides with pain. There
is weak dorsi and planterflexion both feet 3/5 and hypalgesia
both L5
and S1 territories.
Decompressive laminectomy
L4-5 with
foraminotomy both L5 roots. The extruded disc was
removed lateral to the both axillae and
bilateral intradiscal cleaning L4-5 was achieved.
Osteophytectomy of the bony extrusion was
achieved by drilling and osteotomy. The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. Routine closure of the wound.
Smooth postoperative recovery.
The patient showed improvement of the power of
her both feet.
She was sent to the ward.
FOLLOW UP
The patient
came to the clinic 23-August-2003 with clean wound.
SLRS was 30 degrees both sides without pain, but the
weakness of both feet is 4/5 and no sensory
deficit. The patient was advised to continue in
medical treatment and advised to perform
physiotherapy to strengthen the power of the feet.
Comments
There is still an estimated postoperative
recurrence less than 7%, because the disc space
is shallow, but not completely collapsed.
