EXTRUDED DISC L4-5 RIGHT SIDE WITH STABLE RETROLISTHESIS.
Anamnesis
The patient came to the clinic
06-November-2004 complaining of right sciatica
for 2 months with numbness.
On examination: the patient is limping with
exaggerated scoliotic stance. SLRS was 75
degrees in the right with pain. There is weak
dorsi and planterflexion right foot 3/5 with
hypalgesia right L5 and S1 roots territories.
The patient was sent for investigations and MRI
performed the next day showing huge extruded
disc L4-5 right side with stable retrolisthesis.
Right L4-5 hemiflavotomy with foraminotomy of
the right L5 root with removal of the extrusion
and right sided intradiscal cleaning L4-5 disc
space.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. Routine closure
of the wound. Smooth postoperative recovery.
The power of the right foot improved.
He was sent to the ward.
Follow Up
The patient came to the clinic 23-November-2004
with clean wound and neurologically free, except
for the part of the skin were Arabic treatment
was applied to the dorsum of the foot.
Comments
The estimated postoperative recurrence at this
case is around 7%, because the disc space is not
shallow.
