EXTRUDED DISC L5-S1 WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic with history of
LBP for 3 years with exacerbation and left
sciatica for 2 months with radiation to the left
L5 territory. MRI lumbar spine done
21-April-2013 showed extruded disc L5-S1 with
left foraminal occlusion with bulge L3-4 and
L4-5.
On examination is limping with scoliotic stance, SLRS was
20 degrees left side
with pain and weak dorsiflexion left foot 3/5
and planterflexion 4/5. The left AJ is absent.
MRI lumbar spine was repeated and the extruded
disc L5-S1 was bigger than before.
Left L5-S1 hemiflavotomy with
foraminotomy left S1 root. The extruded disc
was reached lateral to the axilla and removed. Meticulous
cleaning of the disc space of L5-S1 from the
left. The bone was bleeding more than usual. It
was waxed.
Smooth postoperative
recovery. The power of left foot became normal.
Comments
The patient still has an estimated postoperative
recurrence rate around 7%, because the disc
space height is still not shallow.
