09-SEPTEMBER-2003  SAMEEH HASAN AHMAD  57 YEARS  SEVERE CERVICAL STENOSIS C4-5, 5-6, 6-7 AND EXTRUDED DISC C7-D1.

 

Anamnesis

The patient came to the clinic 07-Sptember-2003 complaining of neck pain, left shoulder and right upper limb pain for 40 days with weak both lower limbs.

On examination: the patient limping with wide based legs gait. He has pain when looking to all directions more to the left with preserved deltoids. Below this level there is deep tetraparesis with hypalgesia below C5 level. Urination preserved. MRI cervical spine done before the visit showing severe cervical canal stenosis C4-5, 5-6, 6-7 with malacia of the spinal cord.

Discectomy of C4-5, 5-6 and C6-7 was done until the dura was seen along all the posterior aspects. Using Scientx cervical plate bended to align with his natural curve with 8 screws, fusion of C4-5-6-7 was achieved. All stages of surgery were done under C-arm control.

Routine closure of the wound. Smooth postoperative recovery. The power of the upper and lower limbs slightly improved.

FOLLOW UP

The patient came to the clinic 18-September-2003 with clean wound and the power of the four limbs normalized with sensation limited to the left median distribution and left D5-7 dermatomes.

The patient then came 12-October-2003 for check up with X-ray cervical showing good alignment of the construct.

The patient then came 12-April-2015 complaining of numbness both feet. The upper limbs sensation and power became normal, and the power of proximal muscles lower limbs 5/5. There is hypalgesia both feet below the ankle with weak dorsi and planterflexion right foot 4/5. The patient sent for MRI lumbar spine, which performed 12-April-2015 showing lumbar canal stenosis L2-3, 3-4 and L4-5. He was advised for surgery.

 

Comments

The patient has progressive cervical canal stenosis. The sooner the surgical decompression, the better the outcome.
