25-MAY-2004 SAMEER YOUSEF AL-NATOOR 45 YEARS HUGE
EXTRUDED DISC L5-S1 LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came to the
clinic 22-May-2004 complaining of LBP since 1995
with exacerbation of LBP with
left sciatica for 3 months. MRI lumbar spine
done 17-May-2004 showing huge extruded disc
L5-S1 with left downward migration.
On examination, the patient in agonizing pain,
limping with exaggerated scoliotic stance with
weak dorsiflexion left foot 4/5. SLRS was 70 degrees
left side with
pain.
Foraminotomy left S1
root was achieved. Left sided intradiscal cleaning L5-S1 was
performed.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. A free fat was
harvested from the
subcutaneous layer and covered the exposed parts
of the dura and roots to minimize postoperative
scarring. Routine closure
of the wound.
Smooth postoperative recovery. The power of
the left foot normalized.
FOLLOW UP
The patient
came to the clinic 04-June-2004 with clean wound and no motor,
nor sensory deficit.
The patient
then came several times and MRI lumbar done
24-July-2006 showing tiny fragment left side and was
advised to have conservative treatment. He was
neurologically free.
The patient
then came 26-March-2011 telling that after lifting
heavy object last week got left sciatica, with weak
left dorsiflexion left foot 3/5. He was sent for
investigations and MRI done the next day showing
small extruded disc L4-5 left side and he was
advised to keep in conservative treatment.
Comments
There is still an
estimated postoperative recurrence around 7%,
because the disc space is still not completely
collapsed.
