24 AUGUST-2015 SAMIHA HASAN SHIHADEH 55 YEARS
DISKITIS OF L2-3 WITH OSTEOMYELITIS L2 AND L3 WITH INVOLVEMENT OF THE PARASPINAL
MUSCLES.
Anamnesis
The patient came to the clinic 23-August-2015
complaining of LBP with right sciatica for 2
years. Exacerbation of the LBP with numbness
right foot the last month. She underwent left
gluteal abscess drain 2 weeks ago after Voltaren
injection to that area. The CXS of the evacuated
abscess was E.Coli. MRI lumbar spine
performed 22-August-2015 showing diskitis of
L2-3 with osteomyelitis L2 and L3 with
involvement of the paraspinal muscles both
sides.
On examination: the patient is in agonizing
pain. The SLRS was 80 degrees both sides with
full power both legs and hypalgesia L5 and S1
both sides.
Using the C-arm, the level of L2-3 was
identified. A long needle was inserted to the
disc space L2-3 fro the left side using local
anaesthesia. A 3 ml pus was evacuated and sent
for all possible investigations.
The patient was sent to the ward.
Follow Up
The CXS became available 27-August-2015 showed
E. Coli which was sensitive to Amikacin and
Levofloxacin and no tumor, not fungi nor tbc
could be confirmed.
Comments
The patient has infection in the L2-3
area which hematologically came from the gluteal area. All
possible investigations must be considered in mind.
