28-FEBRUARY-2011 SAMIRA SULAYMAN AL-HNETY 56 YEARS
CENTRAL EXTRUDED DISC L3-4 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient
came to the clinic 08-February-2011 complaining
of minimal LBP for 15 years with agonizing right
sciatica the last 4 months.
MRI lumbar spine done
27-December-2010 showing extruded disc L3-4
central with right sided occlusion.
On
examination: The patient is limping with
exaggerated scoliotic stance with SLRS 90
degrees in the right and weak dorsiflexion right foot
4/5 with severe OA both knees.
Using image-intensifier, the L3-4 level was
identified and partial flavotomy L3-4 was
achieved. Foraminotomy of the right L4 root was
performed and the extruded disc was removed from
the right and meticulous cleaning of L3-4 disc
space was done. There was no epidural fat at
this area due to previous severe compression.
Routine
closure of the wound. Smooth postoperative
recovery with normalization of the power of the
right foot.

Comments
The estimated postoperative
recurrence is still ranking around 7%, because
the disc space still not shallow.
