24-DECEMBER-2003 SARA HASAN AL-KOOZ 55 YEARS
EXTRUDED DISC L4-5 WITH SEGMENTAL STENOSIS.
Anamnesis
The patient came to the clinic 21-December-2003 complaining of
LBP with left
sciatica for 5 years with
numbness of the right foot. She was operated for
left knee 3 years ago. After 2 months got
exacerbation of LBP with right sciatica with
further deterioration the last 2 months.
MRI lumbar spine done today showing
extruded disc L4-5 with segmental stenosis.
On examination: The patient is limping with
exaggerated scoliotic stance. SLRS was 30 degrees
in the right with pain and 50 degrees in the
left with pain. There is
weak dorsiflexion and
planterflexion right foot 3/5 with hypalgesia
right L5 and S1 territories.
Decompressive laminectomy
L4-5 with bilateral L5 foraminotomy with
removal of the extrusion. After
that, the roots became lax and right sided
intradiscal cleaning L4-5 disc space was done.
Then bilateral cleaning was achieved.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. A free fat was
harvested from the subcutaneous layer and
covered the exposed parts of the root. to
minimize postoperative scarring. Routine closure
of the wound. Smooth postoperative recovery. The
power of the right foot became normal.
FOLLOW UP
The patient came to the clinic 04-Januuary-2004
with clean wound and SLRS 50 degrees right side
and 80 degrees left side
without pain and no motor, nor sensory deficit.
The patient then came 09-October-2004 telling
that she was admitted to Al-Bashir hospital with
kidney problems 1 month ago and clinical exam
ruled out neurologic deficit and sent for
investigations, but she disappeared.
Comments
The estimated postoperative recurrence is still
ranking around 7%, because the disc space is
still relatively not shallow.
