16-JULY-2014 SARAB ABDEL-RAZAQ AHMAD 42 YEARS
EXTRUDED DISC L5-S1 WITH LEFT UPWARD MIGRATION.
Anamnesis
The patient came to the clinic 15-July-2014
complaining of LBP for 16 months with left hip
pain. The last 4 days progressed left sciatica and numbness all toes
left foot. MRI lumbar spine done 13-July-2014
bad quality
showed extruded disc L5-S1 with left upward
migration.
On examination; the patient in agonizing pain, was limping with
exaggerated scoliotic stance. Positive cough
sign, SLRS was 85 degrees in the right
and 75 degrees with pain in the left. Weak dorsi
and planterflexion left foot -3/5.
The patient was sent for investigations and the
MRI lumbar spine performed the same day, showing
the extruded disc of L5-S1 with left
upward migration. Dynamic studies were normal.
Using MICRODISCXS system with VITOM, left L5-S1
partial flavotomy was done. The
extruded disc was removed lateral to the axilla.
The extruded disc was reaching the left L5 root
and adherent to it. It was removed from that
area. Left sided intradiscal cleaning of L5-S1 disc
space. Routine closure of the wound.
Smooth postoperative recovery. The power of the
left foot improved.
Comments
The estimated postoperative recurrence is
still around 7% because the disc space height is still not
shallow.
Preservation of the ligamentum flavum is
the best way to decrease the scar formation after disc
surgery. This some times is mission impossible and partial
flavotomy is the only available option.
